Dealers at live blackjack cannot see your strategy card on their end. Absolutely use one — it is your right as a player. Basic strategy is not cheating, it is simply optimal play. The house edge of 0.5% assumes you play perfect strategy so using a card is exactly what you should be doing.

Perfect Pairs house edge is approximately 6%. The 21+3 side bet is approximately 3.2% house edge. Both are significantly worse than the main hand at 0.5%. They are entertainment bets only.
